阿爾茲海默癥(AD),這一逐漸侵蝕記憶與認(rèn)知的“隱形殺手”,作為一種神經(jīng)系統(tǒng)退行性疾病,以其復(fù)雜的病理機(jī)制和多樣的臨床表現(xiàn),成為了醫(yī)學(xué)界難以攻克的疑題。β-淀粉樣蛋白(Aβ)和Tau蛋白的異常聚集是導(dǎo)致疾病進(jìn)展的關(guān)鍵機(jī)制之一,此外科學(xué)家們還提出了神經(jīng)元纖維纏結(jié)、神經(jīng)炎癥理論及血管性因素學(xué)說(shuō)等。相關(guān)生物標(biāo)志物的分析檢測(cè)將助力科研工作者更深入地揭示該病癥本質(zhì),為后續(xù)的治療奠定基礎(chǔ)。
β-Amyloid
β-淀粉樣蛋白是阿爾茲海默病中最被廣泛研究的標(biāo)志物之一。根據(jù)β-淀粉樣蛋白假說(shuō),Aβ斑塊的形成是AD病理過(guò)程中的關(guān)鍵步驟,這些斑塊在大腦中的積累是導(dǎo)致神經(jīng)元損傷和細(xì)胞死亡的原因之一。Aβ蛋白是由跨膜蛋白Aβ前體蛋白(amyloid precursor protein,APP)經(jīng)過(guò)β分泌酶(beta-secretase 1, BACE1)與γ分泌酶依次水解產(chǎn)生的含有39~43個(gè)氨基酸的肽段。Aβ42/Aβ40比值作為核心生物標(biāo)志物,能早期、準(zhǔn)確地區(qū)分AD患者與健康人群,其高敏感性和特異性為早期診斷提供了有力支持。
Tau
NF-L
神經(jīng)絲輕鏈蛋白(Neurofilament Light-chain, NF-L)是一種新興的生物標(biāo)志物。NF-L在生理?xiàng)l件下分布于神經(jīng)細(xì)胞內(nèi)部。多項(xiàng)研究證明在患者的認(rèn)知能力下降初期,腦脊液中NF-L水平就已經(jīng)升高,因此NF-L未來(lái)可能發(fā)展成為預(yù)測(cè)臨床前AD癥狀的指標(biāo)。NF-L在其他神經(jīng)退行性病變中也同樣出現(xiàn)變化,所以該指標(biāo)不具備AD診斷特異性,需要與Aβ/Tau生物標(biāo)志物聯(lián)合應(yīng)用于AD的臨床診斷與預(yù)測(cè)。
BDNF
BDNF是一種關(guān)鍵的神經(jīng)營(yíng)養(yǎng)分子,已被證明可增強(qiáng)突觸可塑性并改善學(xué)習(xí)和記憶。在AD的背景下,BDNF耗竭與Tau磷酸化、Aβ積累、神經(jīng)炎癥和神經(jīng)元凋亡有關(guān)。隨著疾病的進(jìn)展,AD患者的大腦,血液和腦脊液(CSF)中的BDNF水平降低。此外,較高的血清BDNF水平與AD患者認(rèn)知功能的改善相關(guān)。
CDK5
CDK5是磷酸化Tau蛋白的主要激酶,其異常激活與神經(jīng)元損傷、tau蛋白過(guò)度磷酸化以及突觸功能障礙等AD關(guān)鍵病理特征密切相關(guān)。
